An Insight into Male Impotence and Cialis as the Drug of Choice
Erectile Dysfunction (Male Impotence)
Erectile dysfunction or male impotence is the inability to
get or maintain an erection that is firm enough for intercourse. The situation,
wherein a person is either unable to get an erection at all or looses erection
during intercourse, is medically termed as erectile dysfunction.
Erection problems are common in adult men as almost all men
at some point in their lives experience occasional difficulty getting or
maintaining an erection. In most people, it is a temporary condition that goes
away with little or no treatment. But, for some it may be an ongoing problem
and thus require medical intervention.
If there is a difficulty in having or keeping an erection
more than 25% of the time, it is considered a problem. The problem can either
be psychological or physical. This can be determined by calculating the number
of nighttime erections. Normally, men have 3 to 5 erections per night, each
lasting up to 30 minutes. In most men, sex drive is unaffected by erection
difficulties. Physical causes are more common in older men, while psychological
causes are more common in younger men. In addition, erection problems have a
propensity to become more common with age, yet, it affects men at any age.
An erection requires the interaction of the brain, nerves,
hormones, and blood vessels. Anything that interferes with the normal
physiological processes can become a problem. Common causes include:
·
Diseases
and conditions such as diabetes, high blood pressure, heart or thyroid
conditions, poor circulation, low testosterone, depression, spinal cord injury,
nerve damage (for example, from prostate surgery), or neurologic disorders
(such as multiple sclerosis or Parkinson's disease)
·
Medications
such as blood pressure medications (especially beta-blockers), heart
medications (such as digoxin), some peptic ulcer medications, sleeping pills,
and antidepressants
·
Nicotine,
alcohol, or cocaine
·
Poor
partner communication
·
Stress,
fear, anxiety, or anger
·
Unrealistic
sexual expectations
·
"Vicious
cycle" of doubt, failure, or negative communication that reinforces
erection problems
Medicines for Erectile Dysfunction Treatment
The use of ED drugs is not recommended in men who have a
recent history of stroke or myocardial infarction (within the last 6-8 weeks),
or who have significantly low blood pressure, uncontrolled high blood pressure,
unstable angina, severe cardiac failure, severe liver impairment or end-stage
kidney disease requiring dialysis. But others can safely use the drug under medical
supervision.
Phosphodiesterase type-5 (PDE-5) inhibitors work by
enhancing the effect of nitric oxide. As a result, the penile cavernosa smooth
muscle relaxes and causes the corpora cavernosa to vasodilate. All these
effects are mediated by cyclic guanosine monophosphate (cGMP). The inhibition
of PDE-5 results in prolonging the effects of cGMP with a subsequent increase
in the nitric oxide concentrations. All three PDE5 inhibitors share this common
mechanism of action and are only pharmacologically active when cGMP synthesis
is activated. The action of the PDE5 inhibitors therefore requires sexual
arousal.
Oral PDE-5 inhibitors have reduced rates of adverse events.
Although, vardenafil, tadalafil, and sildenafil improve erectile function
equally well, the duration of action of the three agents is significantly
different. Sildenafil and vardenafil have half-lives of approximately 4 hours,
while tadalafil has a half-life of 17.5 hours. In addition, the molecular
structural differences are reflected in the pharmacokinetic properties of
tadalafil and its selectivity for PDE isozymes.
Cialis as Impotence Medicine
Sexual problems and male impotence can be a thing of the
past with the intervention of a wonderful drug called Cialis. Cialis is in a
class of medications known as PDE-5 inhibitors, which are used to treat cases
of male impotence around the world.
Cialis acts in the same way as other ED drugs by blocking an
enzyme called phosphodiesterase-5. It results in fewer sides effects and is
taken in lower dosages. Cialis helps the smooth muscles in the penis relax and
widen, which allows for more blood to enter, resulting in faster and easier
erections.
Cialis is an approved prescription medication for the
treatment of mild to severe ED at both 10 and 20mg doses. It should be taken
prior to anticipated sexual activity and without regard to food but sexual
stimulation is needed for it to work.
Cialis is not for everyone. Men on nitrate drugs and
"alpha blockers" should never take Cialis, as these drugs when
combined with Cialis may cause blood pressure to drop to unsafe levels.
